Scheduling for Established Patients

Allergy Shot Patients:

  • Please be aware that all allergy shots must be scheduled in advance; walk-in appointments are not accepted. Make sure to arrive early for your scheduled shot appointment. If you arrive more than 15 minutes late or do not have an appointment, you will need to reschedule.
  • If you have a dual appointment scheduled for an allergy shot and exam, you will need to check in at the front desk for both.

Current Patient Scheduling:

  • For established patients with in-person appointments, please arrive 10 minutes early. Online pre-registration should be completed prior to your scheduled appointment each time.
  • Insurance updates? Please bring the new card in with you so we can update accordingly.
  • When you arrive check in at our front desk.
  • If you arrive more than 7 minutes later than your scheduled time we will need to reschedule.
  • You can now schedule your allergy shot appointments online through our website at any time—just follow the hyperlink on the right.
  • For all other appointment scheduling, please call our office at 541-683-1577, option 3, for established patient scheduling support.
  • You can also access your patient portal via our main page and send us a secure message anytime to schedule or connect with our team.

 

Late Cancellation & No-Show Policy:

As a specialty practice serving multiple regions, we kindly ask that you review our cancellation policy. This helps us provide timely care to patients who may have limited access to Allergy & Immunology services.

We request at least 24 hours’ notice for cancellations. If less notice is given, or in the case of a late cancellation or no-show, a warning letter will be sent as a reminder of our policy.

  • First no-show: Patients will receive a warning letter.
  • Second no-show or late cancellation: A $75.00 fee will be applied.
  • Patients with a history of frequent attendance issues may be subject to scheduling restrictions or dismissal from our practice at the discretion of their provider.
